I have a 16-gauge Sportsman 48 that had the same problem. The dishing happens when the chamber is rough. Get a 12-gauge brush and wrap some fine steel wool around it and chuck it in a power drill. Run it in the chamber for a minute. Now get a 12-gauge bore mop and oil it and add some Flitz to the mop.
